Grizzly ID Request Form
Forgot your Grizzly ID or don’t have one? Complete this form and PACE will contact you with your information.
Registration Change Form
In general, registration should be processed online via your Destiny One account. If you need assistance with registration or withdrawal you may complete this form. A PACE staff member will review and process the request accordingly. For withdrawal, the form may only be used immediately after the course registration is closed and during the next 5 business days. The form must be complete and signed by the participant requesting the add or drop.
Grade Appeal Form
Within thirty (30) business days after the date which the grade was posted, the student shall obtain the requisite Grade Appeal Form. The student shall formulate an appeal in writing, including why the student believes the grade does not reflect the student’s performance based on the criterion in paragraph one of this document, and any supporting documentation. This must be submitted to PACE who will forward a copy to the Program Director.
Allegation of Misconduct Form
To report classroom misconduct please complete the Allegation of Misconduct Form.
Procedure once the form is completed:
- Provide the form to the Program Director and as a backup to the PACE Executive Director.
- Program director will independently meet with the student and instructor to review the complaint within 3 weeks of receiving the complaint.
- After the independent meetings, Program Director will determine the corrective action and provide in writing to the student and instructor within 30 days. Determination is final.
Request for Exception Form
The Request for Exception should be filed for documented extenuating circumstances only (view the form to see the full list). Must be submitted either during or immediately following the course(s) in question. Requests will not be reviewed after one calendar year from the end of the term in question.
