Below are many of the most common forms students will require while attending Oakland University. Some of them require you to be concurrently logged into your Oakland University email account at webmail.oakland.edu. If you are currently logged into a personal Gmail account with this browser, please log out of it and log in to your Oakland University email.

- Change graduate catalog edition (this form is used to change your current graduate catalog to a future graduate catalog)
- Graduate transfer credit request
- Graduate transfer credit request for Oakland University courses
- Petition for course waiver/substitution
- Petition for program transfer
- Petition to repeat a course (for grade improvement)
- Petition for time extension and revalidation of OU credits (master's students only)
- Petition for time extension to complete doctoral requirements (doctoral students only)
- Request for an exception to enrollment policy (located under General Forms)
- Request to exceed maximum credits

- Readmission form
- The Petition for Graduate Readmission is used by the Graduate School for students who were previously admitted to a Master's degree program or Graduate Certificate and enrolled for courses, but have not taken a course at Oakland University within the last two years.
- Readmission is not automatic; deadlines for admission into a given semester may also apply for readmission and additional materials may be required. The graduate catalog current at the time you are readmitted will govern program requirements, policies and procedures.
- Any student who is temporarily unable to attend class or must suspend their studies due to military service requirements will be readmitted with the same academic status they had when last attending the university.
Please schedule a graduate assistantship paperwork appointment. Make sure your calendar is set to the Eastern Time Zone.
For your appointment, come in-person to the Graduate School at 520 O'Dowd Hall, with original, unexpired documents. Please bring the following:
- I-9 employment eligibility verification plus supporting documents for section 2 (see list of acceptable documents on page 2)
- Oath Card
- Confidential payroll information
- Original Social Security Card or Letter from Social Security office
The following documents should be taken to the payroll office at (121 W. Vandenberg Hall):
- Direct Deposit
- Federal W-4
- Michigan W-4
- GA - ESTA notice | English | Spanish
- Electronic W-2
- Substantial presence test (International students only - pick up at payroll office)
We must have these completed forms before you can begin working.
